LETTER OF AUTHORIZATION

They will travel to (city’s name), Dominican Republic on (date of departure) returning on (date of return). (Indicate relationship between authorized person and child).

The flight (number) of (carrier’s name) departs from (city of departure), Canada on (date of departure) at (time of departure). They will be returning on (date of return) at (time of return), via flight (number) with (carrier’s name).

AND I make this letter of Authorization conscientiously believing and knowing that it is of the same force and effect as if made under oath and by virtue of the Canada Evidence Act.

For legalization at the Consulate, please send or bring:

Letter of Authorization plus a photocopy
Two copies of IDs of the authorizing parent(s), child and authorized person.
Two passport-size pictures of the child (school pictures are also acceptable)
Legalization fee is US$75 payable in cash, certified cheque or money order.
If the letter is signed in front of the Consul, no Notary Public signature is necessary and there is not additional cost involved.
If not done in person, please include a pre-paid envelope to return the letter of authorization.
